Dr. Levenson participates in most regional New York plans (CDPHP, MVP, and most Blue plans), many national health plans in the United States including AETNA, United Health Care, Blue Cross & Blue Shield, and Multiplan - as well as most PPO and EPO plans. Please determine if your plan allows for out of area treatment prior to contact.
